A full athletic physical is required every calendar year to participate in sports.  The medical update, steroid consent, and parent permission forms must be filled out and returned to the school each season prior to participating in the first practice.

Annual Athletic Pre-Participation Physical Examination Form

Both Parts A & B need to be turned in.

Part A: Health History Questionnaire - completed by the parent and student and reviewed by the examining provider

Part B: Physical Evaluation Form - completed by examining licensed provider with MD, DO, APN, or PA

Medical Update

Use this form if you have turned in a physical for another sport.  This form is signed by the student and a parent/guardian.

Steroid Consent Form

Must be signed by student and parent/guardian.

Parent Permission Form

Must be signed by parent/guardian.

Banned Substances

The NJSIAA Banned Substances list is for your reference only.  It does not need to be returned to the school.
