Pre-Participation Forms

In order to participate in any athletic season, EVERY Paramus Catholic student-athlete (including Cheer and Dance) must have a physical performed within 365 days of the official start of their respective sport. This physical covers all sports for one year from the signed date of the physical. Only the NJDOE approved physical form will be accepted for athletics. The form can be downloaded or printed using the link below.

After downloading and reading all of the forms below, parents/guardians may complete the online forms here.

Only the Physical Form needs to be printed, completed and submitted to the trainer. All other policies, information and agreements are to be read and agreed to online. Please keep a copy of your original physical at home.

Paperwork must be completed and turned in by the following dates to be eligible for sports: 

August 1
Football, Soccer, Cross Country,
Volleyball, Girls' Tennis
 
November 1
Hockey, Swimming and Bowling
 
November 15
Basketball, Indoor Track, Wrestling
 
February 15
Lacrosse, Spring Track, Baseball,
Softball, Golf, Boys' Tennis

 

Parents please note, if your athlete has a history of Asthma and/or Allergies listed on their sports physical, both the "Asthma Treatment Plan" and/or the "Allergy Emergency Care Plan" need to be completed by yourself and an MD. These forms must be handed in with the current sports physical in order for your child to be cleared for sports.  If your child is on medication for Asthma and Allergies, the trainer, coach, or the nurse must also be notified. In addition to the Opioid Fact Sheet below, all student athletes along with a parent/guardian must watch the Office of the New Jersey Coordinator for Addiction Responses and Enforcement Strategies (NJCARES) and the New Jersey State Interscholastic Athletic Association (NJSIAA) video "Athletes vs Opioids."
